I have been told that people on layoff should hold on to their relia-cards once they get new ones because if there is an extension granted the funds will be put onto the old card. Take the FAN number and possibly your ID ( I did not need ID) to a corporate store, the one in front of Sam's 7612 Rogers Avenue or the one in Greenpointe Shopping Center, past the mall up the hill at 4300 Rogers Avenue.
