From our UAW Benefit
Representative, Paula Betnar
September 2, 2009
IMPORTANT
Paula needs to know if there are any
employees on layoff who have lost or are
about to lose their insurance and have not
been offered COBRA by the company. If
so please call Paula at 479-434-0245.
If you are being offered COBRA there is a
government program that offers assistance at
65% of the cost of COBRA. Please call
Jake Zadny at 479-648-4224 or Paula Betnar
at 479-434-0245.

2009 any employee and their family members

insulin or diabetic medication and/or
supplies).
Employees
are required to use mail-order starting with
the third fill in 2009, (March).
If you use
retail pharmacy, on or after the third fill,
members will pay the lesser of the
mail order co-pay or the full cost of the
medication.
Wal-Mart
fills generic medication $4.00 monthly or
$10.00 for 90 day supply.
The

Caremark;
Our New Drug Cards
Some of our members are having
trouble with our new drug cards. Some are being told at
the pharmacy that we have a $1000 deductible and the meds are
full price until that is met. We have no deductible on our
prescriptions, only co-pays. The phone number on the back
of the card is not helping when you are at the pharmacy.
If you encounter this or any other problem please have this
number ready:
1-800-421-2342.
