Making Your Appointment

Patients are seen by appointment only, Monday through Thursday.

Fri./Sat./Sun. Closed

We know that after school appointments are the most popular. If you are not willing to take your child out of school, we suggest you set up a series of appointments in advance to avoid this problem.

Consultation:

On your first visit, we will create a permanent record. We will ask you too complete a brief information and medical form. This initial visit is complimentary. Please try to arrive a few minutes early and don't forget to bring your insurance card..

Canceling An Appointment:

If you are unable to keep your appointment or are going to be more than 15 min. late, please call the office as soon as possible. This courtesy allows us to be of service to other patients
