 SALES AND HEALTH CONTRACT

         I,_________________________________________________, on this date_____/_____/_____, agree to purchase a (Breed)_________________________ Sex_____, for the sum of $______________.  A non-refundable deposit of $__________________ is payable at time of purchase to reserve/hold the dog.  A balance of $__________________ is due at time of pickup or at 6 weeks for delivered puppies.  Buyer is responsible for shipping, crate, and health cert. costs.

1. HEATLH GUARANTEE:    The purchaser agrees to take this dog to a licensed veterinarian of his choice within 48 hours of purchase.  If at time of examination it is determine that the dog is not in good health due to illness, the cause being clearly attributable to the seller, the purchaser shall return the dog in exchange for a dog of equal value or credit on another dog.  A written diagnosis from the veterinarian must accompany it.  If purchaser does not wish to return the dog at this time, then the purchaser is responsible for veterinary expenses which may occur.  After the 48 hours period, the new owner assumes all further veterinary bills due to illness, missdiagnosis,ect.  The purchaser releases the seller from all future liability upon execution of this contract.  Congenital heart problems guaranteed for 2 years.
    

2. 2 YEAR HIP GUARANTEE:     If the dog is found to be cripple from displaysia, he may be exchanged for a dog of equal value or credit on another dog.  The dog must be x-rayed (evaluated) before 24 months of age, and results reported back to S.C.B within 30 days of evaluation to be valid. 
    

3. Purchase Agreement:     Purchaser agrees and binds himself to take good and responsible care of the dog, feed and house the dog properly and control the do on a lead when off the premises to avoid loss by theft, running away, or otherwise, and promptly to give and secure the best attention to the physical and mental welfare of the dog.  Purchaser agrees the herein described dog will not be cross bred or used for fighting.

Limitations:           This guarantee applies to the original purchaser only.  Guarantee is void if mistreatment, abuse or neglect is evident in the dog.  No replacement will be given if purchased dog has been bred (traceable through registration), neutered or spayed.  Registration papers must be accompany the returned dog to be eligible for replacement dog.  Purchaser is responsible for all transportation cost to and from kennel.
