Central Oklahoma Alliance of Christian Homeschoolers (aka C.O.A.C.H.) Instructions in Making Application for Enrollment 1.) Download
/ obtain current Class Schedule and Teacher Tuition & Fee
Schedule for your information & records. 2.) Download / obtain enrollment packet forms. A.) Family/Student Application Form (Forms A & B should be copied on one page - front & back.) B.) Class Enrollment Form C.) Volunteer Form D.) Driver’s Registration Form (if necessary) 3.) Remit all necessary registration fees: A.) C.O.A.C.H. semester fees (checks made out to C.O.A.C.H.) B.) Materials Fees for each class you are applying to enroll your student(s) (made out to individual teacher) as listed on the Teacher Tuition & Fee Schedule mentioned above. NOTE: THE ABOVE FEES (both COACH and teachers’) ARE NON-REFUNDABLE but will not be processed until the student is actually accepted into the class. 4.) All forms and checks for C.O.A.C.H. registration and teachers’ materials fees will be submitted with the enrollment packet and are subject to: A.) No outstanding balances on tuition, registration, or materials fees accrued by the family during a previous year. B.) Attendance at one MANDATORY Information Meeting for each school term. C.) Received on the designated enrollment dates for new & returning students 5.) Attend the MANDATORY Parent/student orientation meeting on August 14th at Berry Road Baptist Church beginning at 6:30 p.m. Failure to comply with any of the terms outlined above will
result in immediate forfeiture of class enrollment and loss of
