Click here for the Directory Information Form (pdf) (80 KB) that will accompany the September newsletter to update our database and assemble a new directory. Some email programs and browsers may have difficulty with an Adobe Interactive Form so please choose from the following three Options:
- Option 1: Desktop Email Application (Choose this option if you currently use an email application such as Microsoft Outlook Express, Microsoft Outlook, Eudora, or Mail. Fill out the Form and Click the box at the bottom right to Send.)
- Option 2: Internet Email (Choose this option if you currently use an Internet email service such as Yahoo or Microsoft Hotmail. Again, fill out the Form, you will then need to save your Form and return it, as an attachment, to Elaine Penpek using your Internet email service.)
- Option 3: Fill out the Form, then print it out on your printer and snail mail it to:
