I, , a member of the Eglin Yacht Club, request the use of the club for a special function.

The purpose of the special function is:

It is understood the club is open to the membership during this function. RSVP is OR is not requested to allow for food and drink planning.

I will OR will not (select one) be collecting ($ amount) at the door from all members and invited guests.

As the host, I understand that I am liable for any damage. I will ensure the club is cleaned and straightened (chairs & tables back in original positions) NLT the above end time. Lastly as the host, I understand at no time will I allow my guests access to club without myself being present and the club key is to be kept in my sole possession.

Special Instructions
  1. Use fees: $25.00 individual/family, $50.00 club/organization
  2. Club manager must receive this request NLT the second Monday of the month preceding the club use month. This is to allow for board approval and notification of membership. Club manager will notify you of board decision.
  3. Club manager will issue you a club key after receiving proper use fee check made out to: Eglin Yacht Club. Upon completing club cleaning, place key through slot in front door of club.
