Lunch Menu Order Form: The cost of lunch is $2.75 per day and orders will be taken on a monthly basis. Orders must be returned to the classrooms by the 15th of the previous month (For example: lunch orders for October must be sent in by September 15th. If the 15th falls on a weekend, we ask that you have the orders in by the Friday before.) October Order Form (New for 2007)

Certificate Order Form: All you need to do is print the Order Form on a Standard #10 envelope. Once the envelope is printed, mark your selections and send the envelope into school with your payment. (New for 2007)

Event Information Sheet: All Event Chairpersons are required to complete the Event Information Sheet at the completion of the event that they have chaired. Please forward the completed sheet to Kathy Valenti.
