An individualized plan of care to meet the older adult's needs:

WHAT DOES IT DO?
Enables older people to maintain as much independence as possible:
- Reduces loneliness and isolation
- Restores and rehabilitates to the greatest extent possible
- Prevents premature and inappropriate institutionalization
- Preserves dignity and enhances the older person's quality of life
A GOOD CHOICE FOR:
- Older people who need care yet wish to remain at home
- Families who work and cannot leave their older relative at home alone during the day
- Caregivers who need respite from the constant care of a loved one

Older adults age 60 and over who reside in Fairfax County, Falls Church, and Fairfax City
HOURS
Monday through Friday, 7 a.m.- 5:30 p.m.
COST
Daily fee based on older adult's ability to pay
