The second Death Star was created shortly after the battle of Hoth. The reason why it didn't show the Death Star in episode five is because it was over the forest moon of Endor. And remember that star destroyer that the Millennium Falcon was attached on after Darth Vader killed Captain Needa? Well that Star Destroyer went to light speed to the second Death Star to help build it but it wasn't showed in the movie, that part except when it went into light speed but you never saw that Star Destroyer in the fifth movie because it was at the Second Death Star.
