Yaddle
The tiny and quiet Yaddle was only seen is Episode I but I still put her on here because she looks like Yoda's species.

Apprentice: Had an apprentice named Oppo Rancisis who is now also on the Jedi Council
Yaddle was never seen in the movies except for in Episode I when they showed them all at the Jedi Council. She looks like she comes from the planet that Yoda comes from and Yoda and Yaddle's faces look alike. There isn't very much known about this Jedi except she was a very good Jedi Master. I couldn't find out what happened to her after episode II but I think that she might have been killed. But again, I do think that it is impossible to kill Yaddle's species. Remember Yoda was never killed by someone? He just died because he got very sick in episode 6. Yaddle probably was either taken off the council or quit.
