Supportive Home Care Attendant (MR/DD)
Definitions:
Personal Assistant/Em=
ployee: Person
providing direct care to MR/DD individual.
Employer: Person who =
hires
trains and dismisses the Personal Assistant. (In this instance this is the
management of Joshua’s Care.)
Plan
of Care: SRS approved services.
CDDO (Community Devel=
opment
Disability Organization): County appointed organization that works together
with Enrollee/Individual to determine a Plan of Care.
Hours allowed to subm=
it:
Employee should under no circumstances turn in more hours than are allowed
under the current Plan of Care (POC) for each Individual. If the employee w=
orks
more hours than allowed in the POC, those hours will be agreed on and paid =
by a
family representative of the individual receiving MR/DD services. (The only
exception to this would be if we have a private pay agreement with the fami=
ly.
As employer Joshua’s Care LLC
will:
Schedule and pay for =
all
training as required keeping both employee and employer in compliance with
Government Regulations.
Calculate and pay all=
taxes
in compliance with Federal and State codes including (FICA match, SUTA, and
FUTA), as required by law.
Carry on behalf of em=
ployee
and employer, Workers Compensation as well as carry General Liability
Insurance.
Calculate and pay Emp=
loyee wage
for hours of services covered by the Plan of Care less appropriate Federal =
and
State tax deductions. Paychecks will be distributed in a timely manner.
Keep Employee and Emp=
loyer
files as required by law, or for identification purposes.

Joshua’s Care L=
LC as
employer will pay Personal Assistants the agreed upon Medicaid reimbursement
rate for Supportive Home Care up to the number of hours allowed in the POC =
for
each MR/DD individual. We observe six major holidays and will pay the allow=
ed
Medicaid re-imbursement rate for those holidays. Respite hours and emergency
respite hours will be paid at their current rate, also determined by Medica=
id
re-imbursement but holiday pay does not apply to respite hours.
The six major holidays are: New Years Day, Easte=
r,
Memorial Day, Labor Day, Thanksgiving and Christmas.
Payroll Schedule
Payroll is based on t=
he
agreed time period between our client and Joshua’s Care L.L.C. The timesheets will be due based o=
n that
agreement.
Time sheets are to be=
turned
in the first working day after the end of the pay period. When this day fal=
ls
on a weekend or holiday, time sheets are to be turned in the next business =
day.
Time sheets can be faxed to (316)201-1841 or hand delivered Monday –
Friday between the hours of 9am and 5pm to Joshua’s Care offices at 1=

Time sheets that are =
received
by 5pm on the agreed upon day will be processed and checks will be ready for
pick up or mailing on the next business day not excluding any unforeseen
circumstances. When the next working day falls on a weekend or holiday, che=
cks
will be available after 10am the following business day.
For those electing to=
use
direct deposit, time sheets will be processed as above and availability wil=
l be
determined by your financial institutions policies.
Changes in Personal Information
If you have any chang=
e in
personal information (Name, address, withholdings, etc.), please
fax(316)201-1841 or mail written explanation of change prior to time sheet =
due
date. Changes can also be brought in writing to the Joshua’s Care off=
ices
during regular business hours Monday – Friday, 9am to 5pm.

Policies
Suspected Abuse, Neglect or Exploi=
tation
Employees of Joshua=
8217;s
Care LLC are mandated reporters and must report any suspected abuse of an
individual that they serve. Any attendant paid by Joshua’s Care LLC.,=
will
be instructed and agree to contact the CDDO, Caseworker, or Joshua’s =
Care
LLC immediately if they suspect Abuse, Neglect or Exploitation of the consu=
mer
they are caring for. As part of mandatory training, each attendant of
Joshua’s Care will have ANE training quarterly wherein each type of a=
buse
and the appropriate numbers to contact will be reviewed.
If Joshua’s Car=
e LLC.,
or an employee of Joshua’s Care LLC, suspects or receives a report of
abuse, neglect, or exploitation from any party concerning the consumer that
they serve, Joshua’s Care LLC., will immediately report the suspected
abuse to the CDDO, SRS Adult or Child Protective Services, and the
person’s legal guardian, if one has been appointed. If notification of
the legal guardian would be likely to cause harm to the person being served,
Joshua’s Care LLC will then assist CDDO, SRS, or any applicable
government agency in any way requested.
Death Reporting
Any attendant being p=
aid by
Joshua’s Care LLC will be instructed to promptly report to JoshuaR=
17;s
Care LLC and the CDDO any death of a person served by them. They also
understand that they will be expected to report to CDDO and/or SRS any
information related to the death of the person served.
Background Checks
As Required by SRS an=
d in
affiliate agreement with CDDO, Joshua’s Care LLC will do KBI, CANIS, =
the
Adult Central Registry, and KDHE background checks on all employees at the =
time
of hire. Joshua’s Care LLC will also verify a valid Kansas Drivers
License and Insurance for the purpose of transporting persons served.
