Application Form and Agreement for Obtaining Individual grant from Roanoke Valley Tennis Association.(RVTA) 6/25/2005

Introduction.
RVTA occasionally makes small cash grants to junior tennis players who show promise and enthusiasm for the sport.
The grant recipient is usually expected to provide some assistance to RVTA at tournaments or other tennis events.
These grants can be used for such expenses as tennis camp fees, travel expenses, coaching fees, tournament entry fees etc.
Grants are usually approved at meetings of the RVTA board based on the information provided by the applicant on this form and personal knowledge of the applicant by one or more Board members. Of course it goes without saying that the funds need to be on hand in the RVTA bank account !

Please sign the following agreement and have it signed by an RVTA board member (to whom you are not related) who will sponsor your application  :

By accepting this grant (if provided), I agree that the money will be used for the stated purpose. I understand that RVTA has no responsibility in this matter other than in providing the money.

I agree to send to RVTA, before the end of this year, an accountability report on how the money was used and on the success of my efforts.
