In consideration of the acceptance of this application for enrollment into “THE BROWN BEAR EUROPEAN BASKETBALL CAMP” , I/we, intending to be legally bound, hereby for myself, my heirs, executors and administrators, waive and release any and all rights and claims for damages I may have against BUTLER PRODUCTION COMPANY, BROWN BEAR INC. , BRIAN BUTLER, MIKE BROWN, WASHINGTON-LEE HIGH SCHOOL, all sponsors and employees of The BROWN BEAR EUROPEAN BASKETBALL CAMP for any or all damages which may be sustained and suffered by me in connection with my/our association with, or entry into this camp, and which may arise out of my traveling to, participating in or returning from the camp. IF any injury should occur during, traveling to or returning from the camp, I/We agree to pay for all cost present and future, through my/our mediCal insurance policy and/or personal finances. I/we hereby certify that I am a competent, healthy adult assuming these risk of my free will, being under no compulsion or duress. This Waiver and Assumption of Risk is effective from July 12, 2002, to July 14, 2002, inclusive, and may not be revoked, altered. amended, rescinded or voided without the prior written consent of Butler Production Company, Brown Bear Inc., Brian Butler and/or Mike Brown.
