Background - Pre Disease
This is Ben's background before we realized he had a histiocytic disease.Birth was at term without complications to the baby by vaginal delivery. Mom was a VBAC delivery and suffered a uterine rupture after the baby was born. Birth weight was 8 pounds, 3 ounces. He was born at Good Samaritan Hospital, Phoenix on 10/23/01.
He has had no other illnesses or other underlying medical problems. He had one cold with cough for approximately 10 days in February 2002.
No known drug allergies.
Patient exhibited normal growth and development prior to admission. He did have an asymmetry in the left jaw and holding of his head to one side thought to be positional in nature since birth. The child also has a flatness in his head due to the same problem. Patient’s parents report that the baby was overall very happy and never fussy. He exhibited no signs of being in pain or poor health. His appetite was very good and he gained weight quickly since birth. He slept 8 – 10 hours through the night consistently since 7 weeks of age.
He is breast fed almost exclusively. Supplemental formula (Enfamil with Iron) is given to him after he drinks pumped breast milk when mom is away. His mom had returned to work full time in March of 2002. He is fed every 4 hours in the day, and an 8 hour break in the night.
Immunizations were up to date as of 4 months. All subsequent immunizations are on hold. Immunizations to date:
Patient’s primary care doctor is Dr. Diane Matsumoto of Mesa Pediatrics.DtaP/DTP/DT on 1/8/02 and 2/25/02 Polio on 1/8/02 and 2/25/02 Hib on 1/8/02 and 2/25/02 Hep B on 10/23/01 and 11/28/01 PCV7 on 1/8/02 and 2/25/02 Newborn screen 10/24/01 Patient lives with mother, father and 2 ½ year old sister, all of whom are well presently. The whole household had a prolonged respiratory illness with coughing in February.
There are no pets or smokers in the home.
He does not go to day care, but did have care person who is from Mexico since he was born. The child did go to Gymboree play classes since January 2002.
